🌿With the good partnership with Palliative Care Unit of Queen Elizabeth Hospital, we are able to provide continuity of care - seamless care for the patients. This service helps to provide care in the comfort of the patients at their own homes i.e. bringing the hospital to the home of the patients. Thus reducing unnecessary prolonged hospital stay.
🌿The Home Care team responds calls from the patients & families any time of the day whenever assistance is needed.
🌿PCA Home Care looks after all needy patients irrespective of creed, colour, race or religion.
A free support provided by PCAKK in the home of the patient, to enable the patient to spend his/her final days at home amongst his/her loved one
The PCAKK homecare team consist of trained doctors, nurses and volunteers.
✅Confirmed diagnosis of Advanced Cancer or End Stage Organ Failure, or any other life limiting illnesses.
✅Patient has a clear and accessible house address.
✅Patient has a primary care taker at home.
✅Patient lives within the operational area of PCAKK.
🌿Regular home visit to ensure patient and carer are coping with the changing phase.
🌿Focus on adequate pain and symptoms control for patient at home.
🌿Integrates the psychological and spiritual aspects of patient care.
🌿Offers a support system to help family cope during the patients illness and in their own bereavement
🌿End of life of care for patients who wish to be at home with their family.
